APPENDIX C - QUESTIONNAIRE FORMS
PREAMBLE
The following questionnaire forms were used as one of the principle methods of eliciting information from the various jurisdictions related to their forest practices codes and regulations. In most cases, the respondent filled-in the questionnaire and provided this to the report author. In other cases, the questionnaire provided a framework for interviewing the individual(s) representing the jurisdiction, either by telephone or in person.
