You can browse the forms index to find the appropriate PAPER SUPPLY code. For example, if you have two forms you wish to order, one indicated as HVA and the other as HFP, send two orders, one to each contact respectively. Indicate on your order the information contained in the box below.

Ordering Pre-printed Forms from the

Office Products Centre (OPC) Distribution Centre in Victoria

Some ministry forms are available from the Office Products Centre (OPC), Distribution Centre - Victoria, operated by the Queen's Printer, located at 4248 Glanford Ave, Victoria.

The OPC has a web-based, online ordering system. If you have access to this, order your forms and supplies directly. The url is http://pss.online.gov.bc.ca, then select the Distribution Centre - Victoria. You must be "set-up" first before using it. 

To get set-up or to order forms manually, contact their Customer Service Centre by phone at 250 952-4460 or toll free at 1-800-282-7955, by fax at 250 952-4431 or by email.

Otherwise, you can order by faxing your requests using Visa/MasterCard/cheque to the OPC at the fax # noted above. If you have any questions, contact them using the numbers above.

Ordering Publications from the

Government Publications Index (GPI) in Victoria

Some items in the ministry forms index are publications or field guides that are available from the Government Publications Index (GPI), operated by the Queen's Printer, Victoria.

This is a web-based, online ordering system. You can order items from here whether you are a registered user or not. The url is http://pss.online.gov.bc.ca, then select the Government Publication Services. Their phone number is (250) 387-6409 or 1-800-663-6105. 

Some tips. When you use the publication index, do a "search the publications index" for "forests" and enter a part of the form name as the "title" to search for. For example, "juvenile". Or enter the form number into the "ministry reference contains" search field.
