Search Options

  • FULL LIST-- simply click Search, with no options selected, or click Clear, to get a list of all forms by form number sequence.
  • SEARCH BY NUMBER OR NAME
    • Single Word Entry: Entering "39" will return all forms containing the number 39.  Entering "500" will find the FS500 leave form. "Leave" will also return the leave form (the same FS500).  "Silv" will return all forms containing the letters "silv". Don't try "Overtime Payroll on Friday Request Form". It'll never happen!
    • Compound Unconnected Word Search: -- If you are looking for a form called "Emergency Request for Capital Funding", but don't know the full name, you can just key in the words "request funding" (no quotes) and select Search to find this form.
  • LIST FORMS FOR BUSINESS AREA -- to list all forms for a ministry business area, select a business area and click Search. For example, selecting "Revenue" will list all revenue forms.
  • WHATSNEW -- check the box called "Sort by newest forms first" then click Search. This will list the forms by version date sequence. Forms changed or new in the last 6 weeks have a little New indicator.
  • SHOW ALL -- check the box called "Show all fields" then click Search. This will list more data known about the forms. This data is typically not of value to most users, and so, not displayed by default.
  • COMBINATIONS -- enter any combination of the above and click Search. Example: Newest forms under the Revenue business area.
  • PAGES -- after you do a search, you may have more than one page (defaults to 100 forms per page). The Goto, Prev Page, and Next Page take you respectively: directly to, back, or forward a page. Note you can change the page size. Simply enter a number then click Search. Enter, say, 2000, and it will list all of them.

How to Get Pre-Printed Paper Forms (Paper Supply)

  • If the Paper Supply field is blank, pre-printed paper forms are not available. The form is electronic only.
  • If not blank, then pre-printed paper forms can be ordered from the source indicated. Just click on the link and follow the instructions. A "pop up" description of the code for this field is displayed if you hover your mouse over it.

How to Get a Form Electronically

You can either launch or download eforms. The field called Fmt (for Format) is the link to the eform. It also tells you what format it is (such as PDF for Adobe Acrobat format, or DOC for Microsoft Word format). Moving your mouse over it will show a little "pop up" description. This "pop up" works for all the codes so you don't have to remember what they mean.

  • to launch, just click on the link
  • to download, with Microsoft Internet Explorer, click on the link with your right mouse button and select "Save Target As...". A dialogue will pop up asking you where you want to store it on your PC.
