This questionnaire should be completed by the divisional/operation logging manager, divisional forester or equivalent. It may be necessary to have the cost categories addressed by different staff depending upon their area of expertise.Please review the instructions on completing the questionnaire, then complete the enclosed questionnaire and return by courier only to Mike Alexander of KPMG at the address below by February 26, 1997:

Completing the questionnaire

This questionnaire is designed to collect detailed information in respect of individual cost drivers impacting specific logging costs. The information collected will form the basis of interviews to be carried out during site visits at each participating division/operation in early March. The site visits will have the objective of gathering background data to determine whether there is a linkage between specific cost drivers and logging costs and to gather qualitative information on the impacts on individual operations. A final list of the key cost-driver: logging cost interactions will be developed during the site visit.The questionnaire has 7 questions, one for each of the major cost categories below, which are defined in Appendix 2 (not included with this example, see Appendix D).

For each of these cost categories:

1) Review the list of delivered wood cost drivers and cost driver definitions.

2) Identify those cost drivers that explain the 1992 - 1996 cost increases for each cost category.

3) Enter the cost driver or drivers which are appropriate for your operation in the table provided for each cost category.

4) For each cost driver identified, enter a percentage in the table that reflects your best estimate of the percentage of the 1992 - 1996 cost increase attributable to each cost driver (the percentages must add up to 100% for each cost category).
